Adult Foster Care ( AFC )

 

People, Incorporated has added Adult Foster Care (AFC) services to its residential support network to offer another option to individuals who need assistance with medical issues and or daily living skills but wish to continue living an active lifestyle in the community. The AFC program creates a relationship between a family, the individual in need of services, and People, Incorporated’s residential network that is beneficial to all.

The AFC program at People, Incorporated provides nursing care management and case management by a social worker to support the family as well as the individual. The RN and social worker schedule regular home visits to monitor the placement. These staff are also responsible for any training needed by the provider. People, Incorporated also provides twenty-four hour emergency support.

Being able to enjoy the comforts and atmosphere of private home helps the individual remain a part of his/her community while receiving needed supports from People, Incorporated and the provider. The provider takes advantage of extra room in the house or apartment and his/her ability to provide care to earn additional income. Caregivers also receive respite services from People, Incorporated that allow them to schedule vacations, etc.
